Alberta Mentor Foundation for Youth (AMFY) is a not-for-profit organization dedicated to helping junior and senior high school students achieve their full potential through supportive in-school mentoring relationships.

Volunteer Mentor Application Form

The first step in becoming an AMFY mentor is completing a volunteer application form.

Please complete the following on-line AMFY volunteer application form. It will be processed by program staff who will then arrange a personal interview. If you prefer, attached at the bottom is an application form that can be printed out and either faxed or mailed to the AMFY office.

AMFY Mentor Requirements:

The applicant must be 20 years of age or older to apply.
The applicant must be able to mentor a student during regular school hours.
The applicant must have lived in Alberta for at least the past 6 months.
